Quick Guide to the COVID-19 Vaccine Sign-ups
Step 1: Figure out if you are eligible
This list of eligible New Yorkers is constantly growing. The current list for who is eligible can be found here.
You can check your eligibility at the NYS Am I Eligible website.
Step 2: Find a Location
NYC has aggregated all of the public and private vaccination sites at Vaccine Finder. Simply put in your zip code and it will tell you the closest vaccine locations.
Step 3: Make an Appointment
The Vaccine Finder site will tell you whether appointments must be scheduled over the phone or online. Each location will have its own process. Below are a few sites that have aggregated scheduling across multiple locations. If an appointment is not available, keep trying! New availability is constantly popping up as sites are added.
- TurboVax finds the latest vaccine appointments from 43 city and state-run administration sites in the NYC area. You can also follow them on twitter for instant notifications of openings (@turbovax).
- A volunteer-led effort has formed that routinely checks public and private sites on Vaccine Finder and updates with availability. Check out the NYC Vaccine List
- To schedule an appointment online at any of the Health Department locations, go to the Vaccine Pod.
- To schedule an appointment online at any of the NYC Health & Hospital Sites, go to the NYC H&H Vaccination Scheduler.
- NYC has also launched a phone number to schedule a vaccination appointment at a Health Department or NYC Health & Hospitals Location: 877-VAX-4NYC (829-4692)

Step 4: Fill Out the New York State Covid-19 Vaccine Form
After you have an appointment, you will need to fill out the NYS Covid-19 Vaccine Form to bring to your appointment site along with proof of eligibility.
